Effective team leadership and inter-professional approach are some of the most important and essential features of DNP graduates. From enriching such essential, a practicum experience equips a DNP graduates with the necessary clinical experience as well efficient delivery approaches to quality health care. According to the theories clinical practice, quality improvement largely depends on both impact and process theories of utilization and implementation plan. As stipulated by the Lewis theory, the essential of a practicum experience is to offer different learning styles and various viable frameworks for health care provision. Ideally, a practicum experience not only promotes quality improvement and skills of practice but also utilizes inter-professional approach and evidence-based practice in supporting community health and the safety of patients.
The objective of this practicum experience will equip me with evidence based practice, competency and interpersonal skills that are both professional and innovative. As DNP graduate, my area of focus shall be familiarizing myself with nursing practice, health care leadership and management, professional concepts and the changes to effective nursing practice and health. As a learning process, I will focus must time in understanding and evaluating key nursing issues in practicum experience including gathering knowledge on the use of human, technological and financial resources in the provision of health care services. Additionally, I will also apply the principles of evidence-based practice in the appropriate making decision in improving quality care in my practicum center.
In my first one week of practicum experience, I will familiarize myself with practicum setup, organization and leadership structure. During the first few days, i will spend my time with the nursing leader who will introduce me to department heads, administrative heads as well as clinicians and physicians. During the same time, I tour the entire practicum setup to understand and acquaint myself with health care needs and practice currently applied in the institution. This assessment will help me in evaluating the best practice as well as the ways of incorporating professionals, patient and the community in designing and implantation of evidence-based practice. I will also evaluate projects in the practicum setup that are designed to improve prevention, outcome, and management of diseases.
Apart from practicum setup and a project aimed at improving outcome, my other objective in the first week shall be to evaluate the structure of leadership and established a framework for improving patient-centered care and that which effectively address health care provision. In proper consultation with nursing and department head, I will be able to evaluate both health care needs and the projects designed to improve patient-centered care. Moreover, in-depth analysis and evaluation of practicum setup will equip me with the right information on the condition of the patient and the steps taken to raise the quality of health care. In the same line, I will also take time and spend with a couple of the patient and family members to come up with a conclusive nut inclusive evaluation of the practicum setup.
In My second week of the practicum experience, I shall focus on reviewing medication prescription offered by the clinicians, dentist and the physicians as a measure of evaluating the rising issue of polypharmacy, an issue I have come across during my reading. Similarly, I will go through medical records admission and readmission to evaluate prescription history and the statistic of geriatric polypharmacy. I will also evaluate and do an in-depth analysis of the predisposing factors of geriatric polypharmacy. In my second week, I will embark on an assessment of patient needs and the most common health care needs in the practicum setup. I will do this assessment in collaboration with my preceptor and the health care professionals including nurses, clinicians, and administrative heads. During this week a will plan a meeting with the nurses on the various department as well as various head of departments. I will conclude my week by making a visit to various patient wards including pediatric, medical, surgical, oncological and geriatric wards.
In my week four, five, six and seven, I will focus on understanding leadership behavior in practicum experience and both inter-professional and intra-professional experience practiced in the practicum setup with the aim of improving health care outcomes. During this period I will attend, all interprofessional meeting with the aim of applying leadership skills learned in my preclinical classes. Apart from using change theory in interprofessional meetings, I will focus on appraising the leadership expressed by the nursing leader at the practicum setup as well as other administrative leaders of the center. During these meetings, my focus shall be centered in fostering both intra and inter-professional cooperation among nurses, clinicians, dentists, physicians and support staff.
In the last three weeks of my practicum experience, I shall focus my attention on establishing designing projects aimed at improving health care quality in the practicum setup. I shall handle most of the problems and the issues identified in week 3 through seven during week 8 to 10. I will spend the better part of these two weeks reviewing medical list identified in week 7 and 6. Consequently, I will arrange a meeting with the appropriate stakeholders to identify and explore the best intervention and protocols for limiting such conditions. Similarly, I will also identify complications occurring at the practicum setup and the most pressing issues altering patient-centered care. I shall utilize the last week of the practicum experience doing a comprehensive analysis of the entire practicum experience including leadership experience and implementation change done during the eight weeks of the practicum experience.
The practicum experience present nursing leaders the best opportunity to exercise evidence-based practice, leadership and patient-centered care. Additionally, the practicum offers alternative learning styles that are not only patient-centered care but also improve health care outcome. In essence, the experience allows the DNP graduate to explicitly demonstrate theories and frameworks learned in class as well as critically appraise the leadership of the practicum setup. Therefore, the practicum experience is instrumental and critical in developing skills and expertise and offer most alternative learning method to any nursing or medical student.
