This is a main field in the research where the health and clinical sciences which emphases on the care, defense and development of the fitness status of the people and groups. It is a field of training that might be trained in public health or environment health which is defined by WHO as the ecological, communal and economic resources that withstand expressive and bodily wellbeing among the people (CDC, 2013). In this context, medical interventions that come in communities can be grouped into key healthcare, minor health care and tertiary healthcare where each group concentrates on different levels of approach near the public in the United States. In this assignment, my area of focus is the community health needs.
Assessing the health risks in one’s community, one can use the Gordon’s functional health patterns. This is a way which is designed by Marjory Gordon where it provides a platform to be used by nurses in the nursing procedure to deliver a more complete nursing valuation of the patient (CDC, 2013). Apart from this, there are other functional health patterns as designed by Gordon which includes health awareness and health organization, roles and relationships, removal, cognition and perception, sleep and rest, self-perception and self-concept, activity and exercise, nutrition and metabolism among others.
In Gordon’s functional health pattern these systematic approach to data collection enables the nurse to get a determination on the different factors of health management, sexuality and reproduction, coping and stress among others. In this concept the category of nutritional and metabolic is the most vital in my focus area where the community needs are referred to the client’s usual pattern of food and fluid intake. This consists of diet, oral activity health and weight loss or gain. There are researches that shows that the food patterns and fluid consumption adopted in most communities are not appropriate for their health. In most cases, the food eaten, amount of and the frequency of how they take the available foods leads to their health risks. Moreover, the inadequacies of the nutrient supplies should also be evaluated to ensure that they are balanced.
There are resources in the community which assist in community health assessment which are assets in a community that aid in meeting certain needs for those around them. They can be people, places and even structures and also community services which serves a major purpose of gathering precise info illustrative of the needs of a community. The valuations are done prior to taking achievement and in determining current situations and identifying action issues (Afrizal, 2019). The needs valuations also found the vital foundation which are important for development. For completion of assessment, the resources required are home care services which include health care and community support. This are the services delivered at home or non-medical living ability such as elder living public to people recuperating and those that are disabled or those that are seriously ill.
A community has strengths and concerns which assist in assessment that includes, neighbourhood, trust among the members, commitment of people to work, the ability to influence a health policy, experience in community development and attention of internal and external partners among others. The weaknesses and concerns include violence and gangs among the youth and the desperate of funders to become genuine partners. The involved parties should then understand the SWOT analysis of their community (Afrizal, 2019). However there are barriers in implementation of health plans in our community which includes stigma, transportation, confidentiality, legal issues, transportation, liability reimbursement, space and differences in priorities among others. To address these some integration programs should be dedicated to space for program activities and state and federal laws should restrict the sharing of patient information among organizations.
